Phone numbers in pediatric context

Infants don't have phone numbers, of course, so it's natural to want to record a parent's phone number in the infant's (or child's) record. This is OK for a while, but it falls apart when the patient ages into a time of life when they need to be engaged with their own care (often around 12-14 years of age). At that time, for many clinical situations, it's important to differentiate the contact information for the patient from the contact information for the parent or guardian. In the case where the parent's contact information was recorded as the patient's, this is a bit of a mess to clean up. I would recommend, as a pediatrician and a member of Epic's Children's Health Leadership Council, that USCDI promote the practice of NOT recording parental data as the patient's. The data model should, regardless of a patient's age, attach only those data elements that actually belong to the patient. A parent's phone number, for example, should be attached only to the parent's record, which in turn could be associated with the patient's record.

The same logic should be applied to addresses, email addresses, and other contact information.
